What will the role look like?
The role will be based in the city wide social prescribing service Linking Leeds. You will be part of the developing Targeted Social Prescribing Team within Linking Leeds, supporting cohorts of individuals within Leeds. Your aim is to make a real difference to the lives of others and be a keen advocate of the benefits of supporting people with health and social care needs to access support in their community, by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.
What we are looking for?
We are looking for someone who will support individuals to build independence and personal resilience, enabling self –management of their health and wellbeing. This will be achieved according to the person’s individual needs. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access.
This is a fixed term post that runs until the end of March 2026 and we would also welcome applications from people who have less experience but feel they have the skills for the role and share the values of the organisation, and could be developed in the post and gain experience.
